Abstract. Significance. For several decades regulatory documents on calculating standard numbers of endocrinologists’ positions providing outpatient care have not been revised. However, during the last decade there was a significance rise in the number of diabetes mellitus up to 9-10 million cases or 7% of the total population of Russia. A range of chronic diseases requiring endocrinologists’ observation and treatment has widely expended as well. The study purpose: to estimate new standard numbers of endocrinologists’ positions providing outpatient care. Methods and data. To calculate standard numbers of endocrinologists’ positions we used data on standard working hours received from photo-stopwatch observation of 36 endocrinologists in 18 Russian regions in 2015. For calculation we used information from regulatory documents of the Ministry of Labor and Social Protection, Ministry of Health of the Russian Federation, reporting form #30 and data on population size of the Federal State Statistics Service. Results. Currently, there is a compelling need in increasing the number of endocrinologists’ positions providing outpatient care which is confirmed by comparison between estimated and actual number of endocrinologists’ positions calculated based on both regional standards of visits recommended by the Program of State Guarantees and standards of staff positions according to the Order of the Ministry of Health. Currently, the actual number of endocrinologists’ positions is 2.8 times higher (including basic leave) and 2.7 times higher (including basic and additional leaves) than the number calculated using average indicator recommended by the Program of State Guarantees, using numbers of planned duties of the doctor’s position calculated by the authors. The number of endocrinologists’ positions calculated based on staffing standards recommended by the Order of the Ministry of Health shows that it should be increased by 1219 positions to correspond with the occupied positions. Decision about changing the number of physicians requires substantial correction of the planned and standard performance indicators of outpatient care facilities represented in the Program of State Guarantees.
