Abstract. Background. Economic changes taking place in the Russian Federation over the last decades had a significant impact on the level of governmental support of the social services and healthcare system.

Overcoming the economic crisis, the government undertakes all possible efforts to ensure sustainability of medical and social care delivery to the elderly and senile.

Therefore, elaboration of measures on improving organization of medical and social care delivery to the elderly in state and private facilities is crucial and timely.

The purpose of the study is to substantiate the necessity of organizational improvement of medical and social care delivery to the elderly in state and private health care facilities (exemplified by patients with hypokinesia).

Methods. The authors used sample integrated characteristics to describe the study results. The authors also used mean error of arithmetic average (for quantitative parameters) and mean error of relative index (for attributive parameters) to estimate reliability of the integrated characteristics

Results. Defect analysis of care delivery to the elderly in state and private facilities showed that defects in case history, lack of complete description of complaints in state polyclinics added up to 2.4+0.2%, and in private facilities - 2.5+0.1%.

Analysis of satisfaction of the elderly patients suffering from acute chronic diseases and attacks of chronic diseases of different body systems with services provided on the outpatient basis in state facilities showed high association with adequacy and efficiency.

160 (39.6%) out of 404 elderly patients were fully satisfied with medical care; 181 (44.8%) – partly satisfied and 63 (15.6%) were not satisfied.

Comparison of satisfaction with services provided in both state and private facilities showed that the elderly patients paid a special attention to the format and duration of the primary examination and how the doctor communicated with the patient during the first encounter. In state facilities, the elderly patients also paid attention to the format of the final conversation.

Conclusions. The study shows that options to further develop and integrate state and private health care facilities in terms of care delivery to the elderly include development of a unified system of treatment and rehabilitation activities. This system should integrate methods of treatment, prevention and rehabilitation provided by facilities of different types of ownership into unified medical technologies with high population impact.
