Abstract

Significance. Alcohol consumption is one of the causes of delayed care seeking in a life-threatening situation reducing the patient’s chances for a favorable outcome regardless of other conditions. The purpose of the study was to assess frequency of admissions to a multidisciplinary hospital with signs of alcohol intoxication in 2006-2015 and associated timeliness of care seeking based on fatal case study.

Methods. Analysis of 2859 entries in the Register of proceedings of the Hospital Committee on Fatal Outcomes for 2006-2015 in the Arkhangelsk Regional Clinical Hospital and relevant 2859 patient records (Form 003/y) was undertaken.

Results. The share of urgent patients admitted with signs of alcohol intoxication out of the total deaths decreased fourfold from 16.3% in 2006 to 4.6% in 2015. The final clinical (postmortem or forensic) diagnosis of these patients falls under Chapter IX (Diseases of the circulatory system), Chapter X (Diseases of the respiratory system), Chapter XI (Diseases of the digestive system) and Chapter XIX (Injury, poisoning and certain other consequences of external causes) of ICD-10. A large share of urgent patients admitted with signs of alcohol intoxication due to Diseases of liver (K70-K77) and Cerebrovascular diseases (I60-I69) (40.5% and 10.9% respectively), failed to timely apply for medical care.

Conclusions. Results of this study emphasizes high relevance of the problem of alcohol consumption by urgent patients prior admission to the hospital and help to better understand relationship between this risk factor and timeliness of care seeking.

Scope of application. Results of this study can be used to develop regional programs aimed at improving access to medical care in life-threatening situations.
