Abstract. Assessment of prisoners’ health status is of great social significance. True assessment of this or that pathology importance among convicts is a key element, defining volume of intervention needed, aimed at health improvement and health promotion among prisoners.

Purpose. Analysis of the disability adjusted life years (DALY) for 2006-2016 considered as integral indicator characterizing health status of prisoners of the Volga Federal District.

Material and methods. Prisoner’s health status was assessed on the basis of departmental reporting forms, using method recommended by the World Health Organization's for determining disability adjusted life years (DALY).

Results. DALY of prisoners in the Volga Federal District increased from 348.0‰ (men – 347.7‰, women – 373.5‰) in 2016 to 414.5‰ (men – 406.1‰, women – 481.3‰) in 2016. Prisoners’ DALY value was 26.8% higher than the general population index (332.1‰). Over the reporting period the share of years of life lost due to premature mortality in the DALY structure among all prisoners increased from 28.3% in 2006 (men – 29.7%, women – 13.2%) to 35.1% in 2016 (men – 36.6%, women – 21.7%), which is more than three times higher the general population index (12.0%).

The study identified the DALY gender and age structure with the predominant role of certain infectious and parasitic diseases (39.0%), followed by diseases of the respiratory system (19.6%). Diseases of the circulatory system come third (12.6%) followed by mental disorders and behavioral disorders (12.3%). Diseases of the digestive system round out the top five (6.4%).

Conclusions. The obtained data indicate a tendency towards deterioration of prisoners’ health status in the Volga Federal District in 2006-2016. The leading causes of DALY include tuberculosis, HIV infection, acute respiratory infections and successful suicides.
