Abstract. High-tech medical care is the most effective type of medical care based on innovations, resulting in significant and sustainable health improvements and improved quality of patientsĺ life. This type of care is highly demanded by the population; however it is the least accessible one.

Financing of scientific institutions that develop medical innovations is considered the prime objective of the developed countriesĺ budgets.

Relevance of the study is substantiated by the fact that nowadays the Russian Federation faces the risks of decreased accessibility and quality of high-tech medical services at the background of extended insurance principles and methods of financing medical activities of the federal scientific institutions.

The aim of the study is to show up inexpediency of financing of high-tech medical care at the federal scientific institutions through compulsory health insurance.

Methods. Expert review of consequences of the increased influence of the compulsory health insurance system on economics of the federal scientific institutions.

Results. The authors substantiated the necessity to resume the budgetary financing of healthcare innovative sector represented by the federal scientific institutions.

The study also shows that deprivation of the financing functions by the founding shareholders of those institutions can result in their degradation in terms of staffing, infrastructure and intellectual assets. The authors call attention to the fact that federal clinics are characterized by high share of extraterritorial medical care volumes which is financed through standard buffer stocks. Integration of federal scientific institutions into the system of compulsory health insurance could lead to the lack of funds in the standard buffer stock and to substitution of the essential share of innovative medical activity with the routine work.

The authors come to the conclusion that strengthened influence of the compulsory health insurance system on economics of the federal scientific institutions could result in reduced accessibility and quality of high-tech medical care.

Practical implications. Materials of the article could be used to develop and adjust the state policy in the sphere of medical science concerning selection of methods and mechanisms to finance federal health scientific institutions.
