Abstract. Disability, especially in children, is an important indicator of public health and is of social and economic importance besides its medical impact.

The purpose of the study was to analyze prevalence of disability, its structure and dynamics over the last 10 years in children 0-4 years in the Ivanovo region. The study is based on the analysis of annual reports for 2005-2014. Over the past 10 years, the share of children 0-4 years in the structure of child disability in the Ivanovo region has increased. At the same time the prevalence of disability in this age group has decreased, but remained higher than the Russian average. Over the last 10 years the share and prevalence of disability in children 0-4 years residing in the Ivanovo region due to mental and behavioral disorders, diseases of the eye and adnexa and diseases of the musculoskeletal system, injuries, poisoning and certain other consequences of external causes have increased while disability due to diseases of the nervous system, endocrine, nutritional and metabolic disorders, respiratory diseases and urogenital system have decreased. In 2014, in the Ivanovo region the child disability structure was characterized by a higher proportion of congenital malformations, mental and behavioral disorders compared to the Russian average and a lower share of diseases of the nervous system. The findings suggest that the problem of disability in children 0-4 years in the Ivanovo region remains a relevant medical and social challenge that requires systematic monitoring with the analysis of causes and enabling conditions of formation to design an evidence-based approach for developing preventive measures.
